Heparin Infusion Dosing
Based on the SAH heparin dosing sheet. Calculates IBW, adjusted dosing weight (if >130% IBW), initial bolus/infusion, and aPTT titration adjustments. For heparin 100 units/mL.
Patient & Order Inputs
Notes: IBW minimums applied (Male: 50 kg; Female: 45.5 kg). Adjusted dosing weight is used when actual > 130% of IBW: adj = IBW + 0.4 × (Actual − IBW). Initial rates and titration deltas are rounded to nearest 50 units for display. Infusion mL/hr assumes 100 units/mL.

- ACS: Bolus — units (60 units/kg; max 5,000); Infusion — units/hr (— mL/hr) (12 units/kg/hr; max 1,000).
- NEURO: No bolus; Infusion — units/hr (— mL/hr) (12 units/kg/hr; max 1,200).
- PTT ≤ 36: Rebolus — units; increase +— units/hr (— mL/hr); recheck in 6h.
- PTT 37–46: Increase +— units/hr (— mL/hr); recheck in 6h.
- PTT 47–75: No change; if two consecutive therapeutic, check daily.
- PTT 76–90: Decrease −— units/hr (— mL/hr); recheck in 6h.
- PTT > 90: Hold 1h; then decrease −— units/hr (— mL/hr); recheck in 6h.
- Safety: If PTT > 150, hold 1 hour, repeat PTT, and call physician.
